Appointments/Consultations/Cancellations

New patients are always welcome. Our staff will be pleased to assist you and will make all efforts to accommodate your schedule. Everyone at our office is at your service.

With the exception of serious emergencies, it is expected that you keep all of your appointments. If you need to reschedule an appointment we require 24 hours notice. In such a case, please call our office and arrange for a make-up appointment with one of our patient care coordinators. There is a fee for missed appointments.

What to Bring to Your Appointment

A checklist of what to bring to your appointment:

  • Insurance Information
    Please bring your insurance card or necessary insurance information. If your insurance carrier requires a referral, bring referral numbers and forms. If your treatment is covered by Workers' Compensation, bring a letter of authorization from your insurance carrier.

  • Patient Information Form
    You can download forms, print them and complete them before your appointment. Or, you can complete forms that are available in our office.

  • Medical Records
    Please have your medical records from previous physicians transferred to our office prior to your appointment, when possible.

  • A List of All Medications
    Please bring a list of all over the counter or prescription medications, vitamins and herbs that you currently are taking or have taken recently. Include the strength and dosage of each medication.

  • A List of Personal Health Questions or Concerns
    Sometimes we forget all the things we wanted to tell or ask the doctor, so make a list of your symptoms, questions or concerns before you come to your appointment. And don't forget to bring it with you!

  • Should you need to cancel or reschedule your appointment, please notify our office as soon as possible, there is a fee for missed appointments.

Office Hours

Our office hours are 8am - 5pm, Monday-Friday, by appointment only. The office is closed on Saturdays and Sundays.

Refraction Policy

Refraction determines the patient's need for corrective lenses (glasses). However, it can also detect vision loss. Sometimes this loss can be slow, progressive, and go unnoticed by the patient. The test can reveal other conditions that the patient may not detect.

Billing & Payment Policy

The Hill's Eye Clinic accepts cash and personal checks as well as the following credit cards:

  • Visa
  • MasterCard
  • American Express
  • Discover

  • Over 95% of all claims are submitted electronically, on the same day of service
  • If we do not receive a response from your insurance carrier within 60 days, you will receive a statement and will need to contact your insurance carrier regarding payment. The balance due for medical services will be your responsibility
  • Patients are billed monthly
  • All patient accounts over 61 days old are placed with an outside collection agency. Patients are then responsible for their balance and all collection fees
  • We accept: Cash, Checks, MasterCard, Visa, American Express and Discover
  • Checks are electronically debited at the time of service
  • There is a $25 fee for all returned checks

Our office provides on-call service for emergencies. Call 404-284-8288 so that the physician on call can be contacted.

Patient Forms

The following forms can be downloaded to your computer and printed at home. By filling out these forms ahead of time, you will save a significant amount of time during your visit. Please be prepared to provide this information to our office along with your current medical insurance information. If you have any questions when filling out these forms, please complete as much of the form as possible and our staff will assist you with your questions on the day of your visit.
